This is a Registration Form for single-computer users of 
PRIMASOFT Dialer for Windows. Please take your 
time and fill in as much information as possible so that we 
can serve you better. Thank you very much.

If you want to use PrimaSoft Dialer on a network 
you have to sign a site-license agreement with us (it will 
be mailed to you after we receive your order). The price of 
a site-license can be calculated as the following:

  2 to  10 computers:          15$ + (n-  1) x $12.00 = $_____
 11 to  50 computers:         123$ + (n- 10) x $ 9.00 = $_____
